Just a note: Jumping in with the camera as opposed to having it handed to you is a great way to achieve the flooding effect.

Forgetting to install or improperly installing the o-ring is another. :(

I never see that kind of footage in my monitor!! I hope your camera can be repaired.. I never jump in with my housing though..
